Objectives: Hysteroscopy provides optical evaluation of uterine cavity. Hysteroscopy can be performed in diagnostics, transcervical surgery or lymphatic mapping in patients with endometrial cancer. Endometrial cancer is the second most common malignancy of the female genital tract. Its incidence in the Czech Republic is 32/100,000 women and year with a permanent increasing tendency. The most controversial issue is the potential risk of microscopic extrauterine dissemination of cancerous cells within the peritoneal cavity and circulation during hysteroscopy, and the question if the relatively high pressure of distension medium used during...

Objectives: Ovarian cancer comprises almost a quarter of all malignancies of the female genital tract. Its incidence in the Czech Republic is 24 per 100,000 women per year. In comparison with its incidence, the mortality rate remains relatively high, with a mortality ratio of 14 per 100,000 women per year. The five-year survival rate increases very slowly. The aim of our work was to analyse the results by laparoscopy – an “open procedure” that was performed at our department in 2003–2008. Materials and Methods: A total of 24 patients underwent laparoscopy – an “open procedure” between the years 2003–2008...

Introduction: Myomectomy is the golden standard in uterine fibroid treatment. It is, however, associated with a high risk of de-novo adhesion formation. The way to eliminate adhesion formation is to use minimally-invasive surgical approach and antiadhesive barriers. Materials and methods: Analysis of literary resources and data from our department. In our department a total of 27 second-look laparoscopies after myomectomy with application of auto-cross-linked hyaluronic acid gel was performed. 21 patients underwent laparoscopic (LM) and 6 open myomectomy (OM) as the primary procedure. On the second-look intraperitoneal adhesions were evaluated...

One of the most advanced and demanding laparoscopic operations is the pelvic repair. The milestone in the management of pelvic repair procedures is the laparoscopic promontorium fixation. Because of the complexity of the given problem, there is a considerable proportion of complementary complications of the anterior compartment we have to deal with. We are discussing relevant procedures which are aimed to minimize the plausible side effects either as a part of the first operation or as a delayed one. Despite the boom of vaginal mesh techniques, the laparoscopic procedures remain as a gold standard on the field of the advanced urogynaecological...

Purpose: Since 2003 we have used arthroscop as an assisted procedure for ankle arthrodesis. The arthroscopicaly assisted arthrodesis has successively replaced the open surgical procedures used in the past. This procedure seems to be a profitable method and the purpose of the article is an explanation of the principles and bendit of it. Method: We use the standard arthroscopic equipment. We use 2 or 3 ports. After joint mobilisation, debridement and cartilage and bone resection is the correct position stabilised using 3 or 4 screws. After the procedure the ankle is fixed in plaster bandage for 6 weeks and then the excercise and gradually weightbearing...

The current options of laparoscopic pancreatic surgery are presented. Laparoscopic enucleation of neuroendocrine tumours and laparoscopic distal pancreatic resection are already among standard surgical procedures in a number of centres. Laparoscopic pancreatoduodenectomy still remains a selective procedure for only a few world centres.

Radiofrequency ablation is a new, effective and safe treatment modality for Barrett´s esophagus associated with early neoplasia. So far limited experience suggests its potential also for the treatment of early squamous cell neoplasia of the esophagus. Authors report a case of early squamous cell carcinoma of the esophagus treated by several sessions of endoscopic mucosal resection and subsequent radiofrequency ablation.

Natural Orifices Transluminal Endoscopic Surgery (NOTES) is a novel, still experimental approach which uses the body‘s natural openings to access the organs to be operated on. The most common site of access is the gastrointestinal tract wherein an artificial entry into the abdominal cavity is created in the stomach or rectum. Transvaginal or transvesical approach can also be used. The authors mention the history of this method and the current experimental phase both in animals and in clinical trials. They also present an overview of the individual centres in the Czech Republic dealing experimentally with this method. In conclusion, they...

Endoscopic examination methods are widely used in diagnosis and treatment in many branches of medicine. Currently, the size of objects observed endoscopically is most commonly determined by estimation or by comparing to an object of a known size introduced to the observed area for this purpose. The authors present their initial experience with the development of a method allowing direct measurement of the objects observed.
